Countryside Canine Rehabilitation is a referral based facility, welcoming physical therapy referrals from other veterinary hospitals. We value strongly our referral relationship, and foster these relationships by referring our mutual patients back to the pet's primary care veterinarian when physical therapy treatment is completed. Any medical needs, other than related to the assigned physical therapy treatment, are addressed by the pet's primary care veterinarian, unless emergency treatment is necessary or arrangements are made and agreed upon by both hospitals.

How do I set up a referral?
- Call our office upon the recommendation of your hospital, to make a physical therapy appointment.
- Please have your hospital forward us copies of your pet's medical history
- Include with the medical history any blood work results, radiographs and ultrasound photos pertaining to the issue at hand.
What Happens on my First Visit?
At check in, we will ask you to sign an acknowledgement form that indicates that you understand that we are examining your pet strictly on a physical therapy referral basis and cannot accept your pet as a Countryside Veterinary Hospital patient for the period of one year. This strict protocol is in place to maintain the integrity of our referral relationship with your pet's referring veterinary hospital. It is our intention to work hand in hand with your primary care veterinary hospital to provide exceptional physical therapy care to your pet.
How many visits will my pet need?
The number of visits depends on the why your pet is receiving physical therapy, what condition your pet is in, and how well your pet responds to treatment. This will be determined by Countryside Canine Rehabilitation's Certified Canine Rehabilitation Practitioner in conjunction with CCR's Physical Therapy Technician.
